As a Certified Immigration Civil Surgeon, Dr. Juan F Zapata and his team, make sure that all of your medical requirements are met. Our medical staff will help you determine the vaccinations you need and offer all the healthcare services connected to immigration, so this process is as smooth as possible for you and your family. Additionally, we'll make sure you have all the necessary paperwork to complete your application's medical section. After doing an immigration physical examination, reviewing your immunization history, and/or administering vaccinations, We will provide you with a completed Form I-693 to submit with your change of status application.

IMMIGRATION MEDICAL EXAM GUIDE

Do you need to take a medical exam for a green card? Don’t worry, we can help! We are here to guide you to get the immigration medical exams done right, the first time.

ESSENTIAL FOR A FIRST VISIT
  • A valid ID or passport issued by your government.
  • Any and all vaccination records (A translator, is provided if needed).
  • Full payment for the entire exam. Children under the age of 14 may submit alternative identification that includes their name, date of birth, place of birth, parents' full names, and other identifying information.

In the first visit, You will get these tests and activities:

  • Quantiferon (blood draw) (patients 2 years & older).
  • Syphilis blood draw (patients 15 years & older).
  • Gonococcal urine test (patients 15 years & older).
  • Determination of possible vaccines needed.

VACCINATION REQUIREMENTS

According to US law, all applicants for lawful permanent residence must get specific vaccines before their status changes. In accordance with your age, our office will evaluate your vaccination history and administer any necessary vaccinations. Please bring any vaccination records from a previous doctor to your consultation.
